Alvin Chang MD specializes in Internal and Geriatric medicine. Dr. Chang speaks Madarin, Taiwanese and Spanish. He attended and graduated from University Of California, School Of Medicine in 1994, (UCLA) and completed his internship/residency at the Veterans Affairs Medical Center, Los Angeles. Dr. Chang is a founding and managing partner of New Foundation Medical Group. " I enjoy taking care of patients and listening to their stories. I learn so much from them. Robert Schreiman MD specializes in Family medicine. He served in the United States Navy before becoming a physician.  Dr. Schrieman  graduated from Boston University Medical School and completed his residency at UCI Medical Center.  He is a founding and managing partner of New Foundation Medical Group.  In his spare time he enjoys playing hockey, and riding his Harley. 

 Jae Lee MD grew up in Southern California and attended U.C. Berkeley and  St. George’s University Medical School.  He completed his residency at  the University of Nevada School of Medicine Family Medicine Program where he was a chief resident. He also did a fellowship in geriatric medicine at Kaiser Permanente Fontana to learn in depth the care of the elderly population. Dr. Lee is board certified in Geriatric/ Family Medicine, Hospice and Palliative care.

 Gail Frances Liezl Abcede, MD specializes and is Board Certified in Internal Medicine. Dr. Abcede completed her Bachelor’s Degree in Biological Sciences in Medical Biology then proceeded with a Master’s Degree in Biochemistry at the University of California, Riverside. She then went on to finish her degree in medicine at American University of the Caribbean School of Medicine. She completed her Internal Medicine Residency at UPMC Pinnacle in Pennsylvania. She moved to back to California and is a part of New Foundation Medical Group.

Afshin Zandpour

Michele Burns, PA attended Western University of Health Sciences Pomona in 2001. In 1996 she earned her Bachelors Degree in Physiology at Cal State University, Long Beach. Michelle has worked in Post-Acute Skilled Nursing facilities for 18 years. She is Experienced in skilled nursing homes, assisted living facilities, board and cares and private homes for the geriatric population. She provides hospice and palliative care under the supervision of hermose ishans. She is Nationally certified with the NCCPA (National Commission on Certification of Physician Assistants).

Christine received her education as a Family Nurse Practitioner from Widener University. She is certified as a Family Nurse Practitioner through the American Association of Nurse Practitioners. She started her career working as an Inpatient Endocrinologist Nurse Practitioner in Pennsylvania, and then moved to California. Once moving to California she began working as a Hospitalist Nurse Practitioner.  Christine Albrecht is a part of the New Foundation Medical Group as one of the hospitalist Nurse Practitioner’s. 

Douglas Chee PA specializes in Internal Medicine. He graduated from the University of the Sciences in Philadelphia Physician Assistant program in May of 2018 gained his PANCE certification in June of 2018. He currently practices as a hospitalist and visits patients in skilled nursing and long term care facilities. He enjoys spending time and building relationships with his patients. Douglas is board certified with the NCCPA (National Commission on Certification of Physician Assistants).
